INITIAL CONSULTATION:  $300 

The initial consultation consists of a one and a half to two hour appointment including: 

• A detailed review of your medical history and lifestyle challenges
  
  

• A comprehensive physical examination with a focus on the musculoskeletal system
  
  

• Review of any X-rays, MRI’s, and reports
  
  

• Collaboration and decision-making regarding the appropriateness of treatment and individualized therapy approaches

   

TREATMENTS

LIGAMENT AND TENDON INJECTIONS:  $100/ligament or tendon
*NOTE:  Ligament and tendon treatments involve multiple injections along each ligament or tendon but are charged as a single ligament or tendon, not by the number of injections on each ligament or tendon

LARGE JOINT INJECTIONS:  $110/joint (Hip, Knee, Shoulder) 


MEDIUM JOINT INJECTIONS:  $100/joint (Elbow, Wrist, Ankle) 


SMALL JOINT INJECTIONS:  $95/JOINT (Fingers, Toes) 


FACETS, CERVICAL OR THORACIC:  $387


FACETS, LUMBAR / SACRAL:  $335


NEURAL THERAPY (Scar Injections, based on size of scar):  $45-110 


MANIPULATION THERAPIES (Based on number of areas treated):  $50-120 


K-LASER THERAPY:  $65/treatment 




OFFICE VISITS WITHOUT THERAPIES: 
Fees are based on time spent with Dr. Blievernicht at a rate of $300/hour.

* Upon request, we will provide you with a standardized HCFA form with appropriate codes to submit to your insurance company for reimbursement
